PRC Process as of January 2008

All volunteers involved in working with children must complete a Police Record Check (PRC)

STEPS

1. Download and carefully complete a blank PRC form provided on the Ottawa Police website or pick up copy from Kilrea arena canteen. Photocopy 2 pieces of ID, one must have a photo on it. See details on page 2.

2. Give both pieces to CHA Risk & Safety Director or leave in mail slot at Kilrea arena (inside canteen on left).

3. Risk & Safety Director confirms all information is properly filled in.

4. A package of all the Canterbury PRCs is sent to Police Services in the fall (mid-October).

5. Police Services process each form. They send out approved PRCs to the individuals who applied. Those who have a positive hit will also be contacted and told of what to do next (timeframe early January).

6. Individuals must send in a photocopy your approved forms to CHA Risk & Safety Director (by end of January). Please keep your originals.

7. The approved PRC forms will be keeped on file for five years - then we do it all over again.

Remember, returning a copy of your processed PRC to the Canterbury Risk and Safety's mail slot at Kilrea, after receiving the PRC back from the Polices Services, is needed to complete the PRC process.

Any questions or concerns please contact our Risk and Safety Director

Your confidential info will be kept on file.

Thank you for your co-operation!
