Registration
for 2011-12
within
District B – South Ottawa Hockey
For
the 2011-12 season, all new and returning players – whether
they played last season at the house league level or the competitive
B, A, AA, or AAA level – will be required to register with
their home association based on their official place of residence.
Since players
who wish to play this season at the competitive level – which
now includes the Ottawa Jr. 67’s AAA Hockey Club and the Ottawa
Sting Minor Hockey Association (OSMHA) – must ‘make’
a competitive team each year, they too must initially register with
their respective ‘home’ association be it the Canterbury
Hockey Association (CHA) or another eligible association within
the new District B, prior to the start of the competitive association’s
first tryouts. This is an ODMHA requirement to ensure that all such
players are covered by the ODMHA insurance prior to stepping on
the ice.

All
returning players whose 'home' association is the CHA should
receive a personalized registration form with their May 2010 newsletter
containing their pertinent information from the existing registration
system. Any player who wants to register but does not have a form,
please print the CHA Player Registration Form found below.
Any
new players to the CHA (i.e., any player who did not play
for the CHA in 2010-11) may print a blank Player Registration Form
below. Please confirm you live within the Canterbury
boundaries. All new players must include a photocopy of the
player’s birth certificate, or equivalent (e.g. baptismal
certificate), as proof of age. New players also
must complete a Code of Conduct form, Declaration of Residency Form
also found below.
